Department Information

Job Information

This position provides case management to adults with psychiatric disorders, addictions or dual diagnosis, reports to the Regional Manager, Mental Health and Addiction Services. This position is responsible for: Intake, assessment and referral of clients to Dehcho Regional and Territorial services, Case management of clients accessing out of territory placements. The provision of outreach services to a caseload of additional clients residing independently requiring information, support, supervision, referral to other community resources and related joint case-planning with other agencies; Playing a key liaison role with non-government organizations providing supported housing and day programming. Participation on behalf of NTHSSA at key joint stakeholder meetings, Formation of and management of assertive treatment or wrap around teams to support high needs clients in the community.

Typically, the above qualifications would be attained by:

A Master’s Degree in Social Work, A Bachelor’s Degree in Social Work or psychiatric nursing with 2 years of related experience; A Bachelor Degree in a related social science discipline with 3 years of related experience; A Diploma in Social Work with 4 years of related experience.

Salary Information

 The salary for this position starts at $42.75 per hour (approximately $83,362. per annum) plus an annual Northern Allowance of $10,192.
