Territorial Specialist, Chronic Disease Management

Located in Yellowknife, the Territorial Specialist, Chronic Disease Management (Specialist) reports directly to the Territorial Manager, Primary Care and is responsible for the provision of leadership, training, planning and support to chronic disease management across the spectrum under this portfolio, and will help guide practice based on the shared vision of the NTHSSA. The Specialist will be called upon to address a variety of issues on a daily basis ranging from addressing client's complaints, providing briefing notes to the Director of Health Services, providing advice and direction to other Territorial Leads, Territorial Specialists, and Regional Managers, and representing the NTHSSA in meetings, or conferences. This position will be required to foster and sustain academic partnerships to advance professional practices and develop the work force as a territorial lead. The Territorial Specialist, Chronic Disease Management will role-model vigilance in ensuring the highest of quality care.
Typically, the above qualifications would be attained by:
Baccalaureate of Health Related field plus (5) years of recent clinical health care experience.

The salary range for this position is from $50.95 per hour to $60.85 per hour (approximately $99,352 to $118,657.) plus an annual Northern Allowance of $3,700.
