Calling All Qualified Home Care and Case Manager Professionals! 

We are seeking dedicated, hard-working professionals with a passion for providing exceptional support in the areas of homecare and end of life care. Our team members enhance the lives of individuals and their families with compassionate and fulfilling care based in the home.

In this role you’ll be an independent worker as part of a larger team that specializes in providing palliative and advanced wound care among other services to clients. A rotating day/evening shift schedule begins with a morning huddle after which you will be responsible for managing your day by scheduling five to seven daily client visits.  
To be successful in this role you will need the ability to work independently while being an eager team player, as part of a multidisciplinary team. Looking at the whole of the client’s health needs holistically, utilizing your critical thinking skills.

Salary Information

The salary ranges for these positions range from $40.91 per hour to $58.68 per hour (approximately $79,775 - $114,426) plus an annual Northern Allowance of $3,700.
