Job Information

Reporting to the Regional Manager, Continuing Care Services, the Relief  Regional Supervisor, Continuing Care Services, is responsible to administer the Home Support program and provide clinical support, coordination of ongoing education, and program development for the Home Support Program. Home Support services include personal care, home management support, rehabilitation services, respite, wound care, and palliative care.

You will be responsible to assist, coach and mentor Home Support Workers; ensuring optimal care is provided to clients. Your team directly impacts the physical, social, and mental health of the client and their families, and facilitates clients to remain in their home longer, maximizing their independence. 

Approximately 50% of your day will be spent providing care in the community, and the other half of your day will be spent on administrative duties within an office setting. Under the direction of the Regional Manager, Continuing Care Services, you will research current trends, complete a community needs assessment, plan for service delivery and programming, maintain statistical data and monitor and evaluate the Home Support program to meet the needs of clients within the Yellowknife region.

KNOWLEDGE, SKILLS AND ABILITIES 
• Ability to read and write English.
• Be computer literate and able to use excel, word, other computer programs.
• Valid class 5 driver's license and immediate access to a reliable motor vehicle.
• Excellent communication (oral and written), public speaking and teaching skills. 
• Knowledge of the community and its resources and be culturally sensitive.
• Ability to work in a multidisciplinary setting.
• Ability to work independently with minimal supervision. 
• Ability to problem solve independently.
• Ability to work shift work e.g. evenings and weekends. 
• Ability to supervise and evaluate other employees. 
• Ability to commit to actively upholding and consistently practicing personal diversity, inclusion, and cultural awareness, as well as safety and sensitivity approaches in the workplace. 


Typically, the above qualifications would be attained by: 
• Grade 12 education.
• Post- secondary personal care attendant or long term care attendant course.
• Five years recent experience in a home or community health care setting as a home support worker.
• One year recent supervisory experience required.
• Current CPR certification and maintained every two years. 
• First aid current and maintained every three years.

Salary Information

The salary for this position is from $40.91 per hour to $48.86 per hour plus a pro-rated annual Northern Allowance of $1.90 per hour.
