Job Information
As the Cancer Nurse Navigator (Navigator), you will be focused on supporting enhanced access to cancer support services to both external providers, clients, and families by acting as a single point of contact during key transitions points along the cancer care continuum, including: initial connection and access to cancer care at initial diagnosis, transitioning to active treatment, transitions at end of treatment (palliative care, survivorship, return to primary care), and acting as a key contact to re-establish connection to cancer care for providers.
Working collaboratively with Territorial, Regional, Community Health Centre sites, visiting specialists and facilities outside the NWT, you will be an integral part of the interdisciplinary team providing a single point for coordination and integration of services for the client during key transition points. You will engage in the coordination of safe, integrated, comprehensive, and individualized care across the cancer care continuum, and will provide and educate clients on survivor care plans.
You will promote a holistic and comprehensive approach to planning, organizing, teaching, and relationship development that will best meet the needs of the client; acts as an advocate, facilitating communication between the clients, their family, Family Practitioners (FPs), specialists, and other healthcare professionals; engages in direct cancer patient care during various inpatient and outpatient departments at Stanton Territorial Hospital (STH); respects the client and family through supporting and maintaining confidentiality of client information; and recognizes the supportive association within the cancer care trajectory to promote emotional self-care.
Typically, the above qualifications would be attained by:
A Baccalaureate Degree in Nursing and a completion or willingness to complete an oncology nursing program; minimum of 5 years nursing experience; minimum of two (2) years Oncology nursing experience preferred.

Salary Information
The salary for this position starts at $49.12 per hour (approximately $95,784 per annum) plus an annual Northern Allowance of $3,700.
