Self-isolation plan

Some travellers will be required to self-monitor rather than self-isolate, or have reduced self-isolation requirements. All travellers entering the NWT must file a self-isolation plan, regardless of vaccination status

Self-Isolation Plan (regular)

NWT residents and non-residents travelling into the NWT for non-work purposes, other than individuals travelling from Nunavut, needs to complete this self-isolation plan. Non-residents of the NWT also require a travel exemption.

Self-Isolation Plan (Nunavut travellers)

NWT residents and non-residents who have been in Nunavut and/or the NWT for at least the past 14 days. Non-residents of the NWT also require a travel exemption.

Self-Isolation Plan (worker)

NWT residents and non-residents travelling into the NWT for work purposes need to complete this Self-Isolation Plan, unless you are a supply chain, flight crew or airline worker and will be in the NWT for less than 36 hours.

After you submit a plan

Public health officials will review your self-isolation plan and get back to you.
