Key Responsibilities
- Provide comprehensive nursing care in the community, including chronic disease management, acute care, rehabilitation, and palliative care
- Assess, plan, implement, and evaluate individualized care plans for clients and families
- Coordinate services and collaborate with interdisciplinary healthcare teams
- Deliver health education and promote wellness, self-care, and disease prevention
- Support clients with activities of daily living and ensure safe home environments
- Facilitate referrals to appropriate health and community resources
- Maintain accurate clinical documentation and client records
- Contribute to program development, quality improvement, and evaluation initiatives
- Provide leadership, mentorship, and guidance to team members and students
- Participate in community outreach, staff training, and interdisciplinary meetings
Key Qualifications:
- A Nursing Degree AND
- Two (2) years of recent acute nursing experience in a medical, surgical, or home care setting.

Labour Market Supplement
The successful incumbent will receive a $11,000 supplement upon hire and an additional $11,000 supplement after completion of 12 months of continuous employment contingent on remaining employed in an eligible position for a further 12 months. This is a taxable benefit and will be subject to applicable deductions.

Registrations
Eligible for current registration with College and Association of Nurses of the Northwest Territories and Nunavut (CANNN) is mandatory for this position.

Salary Information
The salary range for this position is from $52.00 per hour to $62.09 per hour (approximately $101,400 - $121,076.) plus an annual Northern Allowance of $7,280.
