The Northwest Territories Health and Social Services Authority (NTHSSA) is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home and Community Care Team in Fort Smith. This rewarding position provides an opportunity to support clients in their homes, helping them maintain independence, manage chronic conditions, and receive quality care close to where they live.
As the Home and Community Care LPN, you will use the nursing process assessment, planning, implementation, evaluation, and documentation to provide comprehensive nursing care to clients across all age groups. You will work collaboratively with Registered Nurses, physicians, home support workers, and other healthcare professionals to deliver client-centered care and promote positive health outcomes.
Key Responsibilities:
- Conduct client assessments and monitor health status.
- Develop and implement care plans in collaboration with the healthcare team.
- Administer medications and treatments within scope of practice.
- Provide wound care, catheter care, ostomy care, and chronic disease management.
- Support palliative and end-of-life care services in the community.
- Deliver health education and reinforce established teaching plans.
- Participate in the Home Care Foot Care Program.
- Respond effectively to emergency situations, including CPR and anaphylaxis.
- Maintain accurate electronic medical records and clinical documentation.
- Collaborate with clients, families, physicians, and other healthcare providers to ensure coordinated care.
Key Requirements:
- Licensed Practical Nursing Certificate (full scope).
- Advanced Assessment and Medication certification, including Intramuscular and Subcutaneous injections.
- Minimum of two (2) years of recent LPN experience in acute care, medical, surgical, or home care settings.
Additional Requirements:
- Valid Class 5 Driver's License
- Eligible for current Registration with CANNN

Labour Market Supplement
The successful incumbent will receive a $11,000 supplement upon hire and an additional $11,000 supplement after completion of 12 months of continuous employment contingent on remaining employed in an eligible position for a further 12 months. This is a taxable benefit and will be subject to applicable deductions.

Salary Information
The salary range for this position is from $45.34 per hour to $54.15 per hour (approximately $88,413. - $105,593.) plus an annual Northern Allowance of $7,7273.
