| Mon, 08/26/2019 - 15:50 by laurel_mcdonald May 23, 21:28-Officer McDonald found staining on the right side of the fuel tank. Checked the valve on the top of the fuel tank, valve was open. Officer McDonald closed the valve. The tank was tilting to the right on a angle.
Officer McDonald can hear dripping underneath the building, a portion of the skirting was already removed, officer mcdonald can see dripping from underneath the building in multiple places, possible pipe busted in the building
21:34 Officer McDonald called the Yamouria Inn Manager and left a message, is the yamouria Inn responsible for the dental building, 21:45 Yamouria Inn manager advised he is not responsible for the building. Officer McDonald contacted Peter Guther 403-589-2744 and asked if the Yamouria Inn is responsible for the dental building, Mr. Guther advised they are not responsible for the building. Officer McDonald asked if they had access to the building, he may still have the key but would have to look for it.
24 May-Officer McDonald called SRP Petroleum and spoke with Alexis Peachy regarding the last time the fuel tank was filled at the dental building. January 7, 2019 seven liters was added to the tank. Ms. Peachy advised officer McDonald that she contacted the manager, Linda Smith at Western Arctic Dental, that something might be wrong with the fuel tank. Ms. Peachy never received a response.
Officer McDonald contacted Kim Welsh at Western Arctic Denta, 867 777 3008 to advise of the situation with the tank and the unknown issued under the dental building.
May 27-Officer McDonald contacted Kyle at Northridge Contracting regarding the spill site. No scope of work has been completed. No approval from Western Arctic Dental
June 19-Offier McDonald contacted Kim Welsh regarding site. Nothing has been completed at the site. Dr. Victor Dorkihine is part owner of the building. Kim spoke to businesses in town regarding remediation.
Either the tank was overfilled or whom ever installed the tank, did not do it properly. Peter Guther is the one looking at the building. Officer McDonald was advised by Mr. Guther that a contract was not renewed with Western Arctic Dental.
15:33-Dr. Victor Dorkihine advised Officer McDonald the inspectors have no problem with the fuel tank. No progress on the site. Dr. Victor Dorkihine will be evaluating the situation himself. Call to Western Arctic Dental from SRP regarding issue and it was rectified. Officer McDonald advised that a spill report is required.
