The Minister of Health and Social Services has released the 2019 NWT Patient Experience Questionnaire. The Questionnaire is available in both print and online formats.

The Questionnaire is designed to assist the Department of Health and Social Services to understand the quality of care we are providing. Responses help the Department improve services and monitor progress over time.

Questionnaires are available at all health centres and hospitals. Online versions of the Questionnaire can be found at www.hss.gov.nt.ca. Responses can be completed anonymously and are considered confidential.

The Questionnaires will be available for all residents using health care services. The Department of Health and Social Services will receive completed Questionnaires from February 25, 2019 until March 25, 2019.
