The Department of Health and Social Services (HSS) is currently conducting a review of Supported Living (SL) services provided to adults with disabilities.
HSS would like feedback through a survey on individuals’ experience with, or need for, supported living services. The information collected will inform the department about how to best meet the needs of NWT residents in the future.
Supported Living (SL) services provide assistance to adults with disabilities, enabling them to live as independently as possible, thereby providing opportunities for community participation and inclusion.
NWT persons with disabilities, and their families and caregivers, are encouraged to participate by completing the confidential Supported Living survey until November 17, 2021. Residents can complete the survey:
- Online through the GNWT’s public engagement website at: https://www.surveymonkey.com/r/8JRB5HB; or
- By obtaining a paper copy of the survey available through Government Service Officers (GSO) in NWT communities.
