The Public Health Nurse Practitioner (PHNP) is a key clinical leader in delivering holistic, culturally responsive, and evidence-based care to the residents of Yellowknife, N'Dilo, and Dettah. Working with the Northwest Territories Health and Social Services Authority (NTHSSA), you'll provide advanced nursing and primary care services that blend prevention, health promotion, and treatment, improving the health and well-being of individuals, families, and entire communities.

In this vital role, you'll take the lead in disease prevention, outbreak management, and chronic disease care, while fostering partnerships that strengthen northern public health. As an autonomous practitioner, you-ll have the opportunity to use your full scope of practice to assess, diagnose, and manage care, all while mentoring fellow nurses and supporting community outreach and education. The PHNP is instrumental in bridging public health and primary care , promoting recovery, resilience, and health equity across the Northwest Territories.

Key Duties and Responsibilities
    - Provide advanced health assessments, diagnoses, and treatment within the Nurse Practitioner scope of practice.
    - Deliver and evaluate public health programs aimed at preventing and reducing the impact of communicable and chronic diseases.
    - Lead case management, disease surveillance, outbreak response, and vaccination programs.
    - Provide clinical support across maternal/child health, chronic disease, and sexual health programs.
    - Design and implement community-based health promotion and harm reduction strategies.
    - Collaborate with individuals, families, and community partners to improve access to preventative and primary care services.
    - Mentor and support public health nursing staff, students, and other healthcare professionals.
    - Contribute to policy development, quality improvement, and research initiatives within the NTHSSA.

Typically, the above qualifications would be attained by:
This level of knowledge is commonly acquired through the successful completion of a BScN or BN nursing degree program with two (2) years of nursing experience and two (2) years as a Nurse Practitioner in a public health or community health environment, along with active registration and certification as a Nurse Practitioner.

Additional Requirements
    - Eligibility for registration with CANNN
    - Class 5 driver-s license and a satisfactory criminal records check.
    - Ability to obtain and maintain current certifications including:
        - Non-Violent Crisis Intervention
        - NWT Immunization Certification
        - CPR, WHMIS, Fire/Disaster Training, Hand Hygiene, Infection Control Practices, and Fit Testing
        - Breastfeeding Course (20 hours)

Labour Market Supplement

The successful incumbent will receive a $10,000 supplement upon hire and an additional $10,000 supplement after completion of 12 months of continuous employment contingent on remaining employed in an eligible position for a further 12 months.   This is a taxable benefit and will be subject to applicable deductions

Term Position

This job opening is for a one (1) year Term.
